CNC INFECTION

Central nervous system (CNS) infections referred as infections involving the brain, spinal cord, optic nerves, and or their covering membranes (meninges). They are medical emergencies that are associated with substantial morbidity, mortality, or long-term sequelae that may have catastrophic implications for the quality of life of affected individuals. They are broadly categorised into meningitis, encephalitis, and abscesses.
Meningitis is an inflammation (swelling) of the protective membranes covering the brain and spinal cord. A bacterial or viral infection of the fluid surrounding the brain and spinal cord usually causes the swelling. When it caused by tuberculosis bacilli it is called as TB meningitis, similarly fungal infection is called fungal meningitis. However, injuries, cancer, certain drugs, and other types of infections also can cause meningitis.

Tuberculous meningitis (TBM) is caused by the seeding of the meninges with the bacilli of Mycobacterium tuberculosis (MTB) and is characterized by inflammation of the membranes (meninges) around the brain or spinal cord. Approximately one-third of the world's population is presumed to be infected with MTB.
Fever and headache are the cardinal features; confusion is a late feature and coma bears a poor prognosis.
Diagnosis of TB meningitis is made by analysing cerebrospinal fluid collected by spinal puncture.
The recommended treatment regimen for presumed drug-susceptible TBM consists of two months of daily INH, rifampin (RIF), pyrazinamide (PZA), and either streptomycin (SM), or ethambutol (EMB), followed by 7–10 months of INH and RIF.
